Terms of Service for EZ Report

EZ Report is a simple and secure platform for submitting concerns anonymously or with your name. It’s designed to help individuals speak up safely and control how their reports are shared and escalated. By using EZ Report, you agree to the following terms.

1. Platform Purpose

EZ Report allows users to submit and manage reports, including optional contact information, attachments, and routing preferences. Our mission is to support transparency, safety, and user empowerment without requiring accounts or personal data unless you choose to share it.

2. Eligibility

EZ Report is intended for users aged 13 and older. By submitting a report, you confirm that you meet this requirement.

3. Content Responsibility

• You are responsible for the content you submit.
• You must not submit false, defamatory, threatening, or illegal material.
• You must not impersonate others or misrepresent facts.

4. Anonymity & Contact

You are not required to provide your name or contact information to submit a report. If you choose to include contact info, it may be used solely to respond to your report or manage escalation, as configured.

5. Escalation & Routing

Users may choose recipients for their reports, and optionally configure automatic escalation (e.g., if no response is received within a set time). You are responsible for ensuring accuracy of recipient emails or escalation settings.

6. Data Use, Storage & AI Processing

• Reports are stored securely with encryption in transit and at rest.
• Data may be retained for a minimum of 5 years or longer based on legal or operational needs.
• EZ Report may use automated tools, including artificial intelligence (AI), to summarize reports, extract insights, and gather public information about organizations or individuals referenced within submissions. This helps improve clarity, routing, and response time.
• EZ Report does not share your data unless you explicitly designate recipients or if required by law.

7. Prohibited Use

You may not use EZ Report to:

• Threaten or harass others
• Commit fraud or impersonation
• Distribute malware or manipulate the system

8. Service Changes

We may modify, suspend, or discontinue parts of the service at any time. We will provide notice when feasible, especially if changes may affect access to your reports.

9. Termination

We reserve the right to remove content or block access for users who violate these terms. Users may request deletion of their report at any time using their case number or linked contact method, if one was provided.

10. Liability Disclaimer

EZ Report is provided "as-is" without warranties. We are not liable for damages resulting from misuse, unauthorized access, delays, or system outages.

11. Third-Party Tools

EZ Report uses services such as email providers, storage vendors, and encryption tools. These services operate under their own terms. We only use reputable providers with strong security practices.

12. Legal Jurisdiction

EZ Report is operated by People Health and governed by the laws of the Commonwealth of Virginia. Any legal disputes must be resolved in the state or federal courts located in Richmond, Virginia.

13. Changes to Terms

We may update these Terms periodically. Changes will be posted to this page with the updated effective date. Continued use of EZ Report means you accept the new Terms.
